New Patient Registration
Cator Medical Centre welcomes new patients.
If you need to register with a doctor and are living in the surgery catchment area, you can visit the surgery during the surgeries opening hours to collect your registration documents. Alternatively you may print the registration form and the new patient health questionnaire using the links below.
Under 5's registration - additional information
Please note - there is a separate health questionnaire for under 5's as it is important we obtain full immunisation history prior to registration.
The registration form must be completed in full and then handed in at reception. A relative or friend can fill in the form with your consent.
While your registration is being processed we need to confirm that you are who you say you are. Please bring along with you two forms of identification, 1 to be photographic.
- Some examples of the type of identification documentation are:
- Driving Licences
- Utility Bill
- Phone Bill
- Bank Card / Bank Statement
- Rent book or tenancy agreement
- Council Tax bill / Council rent book
Once you have completed all the necessary documentation, your registration will be processed by our administrative team. Your medical records will be transferred to us from your previous doctor. If you have not had a doctor in the UK before, a new medical card will be made for you with a unique National Health Service (NHS) Number.
